What size of property is Phoenix House?
We have three self contained, large suites and one studio apartment which is ideal for respite care.
How are care plans compiled?
Based on assessment of needs and risks, care plans are drawn up with the Individual or, with consent of their representative(s). The care plan offers a comprehensive holistic evaluation of the Individual but principally focuses on the health and care needs and how best these are to be met. The individual care is planned to use the Roper Logan and Tierney model of needs to assess the activities of daily living. In conjunction with this care plan a host of other assessment tools are used to deliver the best care for the Individual. Care plans are reviewed monthly but should there be a change in supporting care this will be done more frequently.
What is your admissions procedure?
On receiving a referral, we will arrange to undertake an assessment to ascertain the care needs of the individual. This will be carried out either at their current address, hospital or at the Home. We encourage Individuals and relatives to visit the Home as the first step of the assessment process. An Individuals Guide and a Statement of Purpose is also available. The completion of the Assessment form is done before the Individual is admitted to the Home. We only offer a place if we can be sure we can provide the support to the individual. All residents are given a Residents Agreement, together with the Terms & Conditions of Residency
Do you encourage social activities?
Our Team organises a bespoke variety of activities to suit the needs of the Individual/s. These are displayed on the activity board and can be subject to choice making by individuals. We at Phoenix House feel this is a crucial element of independent living. Participation in social activities, depending on the individual needs and wishes, are actively encouraged.
What sort of meals are provided to service users?
All individuals will be encouraged, with support, to prepare as much of their own meals and drinks as possible, dependent on ability. The menu is displayed on the dining room notice board, and we have a choice of meals on a daily basis.
Can family members or guardians visit service users?
Yes, we actively encourage social visits for all service users, subject to ID checks and signing in upon arrival.
